Pancreatic Diabetes

Pancreatic Diabetes, also known as Type 3c Diabetes or Pancreatogenic Diabetes, arises from diseases or damage to the pancreas that impair its ability to produce insulin. Unlike Type 1 or Type 2 Diabetes, this form is caused by an underlying pancreatic condition and requires a distinct approach to management.

Common Causes

  • Chronic pancreatitis (long-term inflammation of the pancreas)
  • Acute pancreatitis
  • Pancreatic cancer
  • Surgical removal of part or all of the pancreas (pancreatectomy)
  • Cystic fibrosis affecting the pancreas
  • Haemochromatosis (iron overload)

Symptoms

  • High blood sugar levels (hyperglycaemia)
  • Increased thirst and frequent urination
  • Unexplained weight loss
  • Fatigue
  • Abdominal pain
  • Diarrhoea or fatty, greasy stools (due to enzyme deficiency)
  • Episodes of low blood sugar (hypoglycaemia)

Diagnosis

  • Blood glucose and HbA1c testing
  • Pancreatic imaging (CT scan, MRI, Ultrasound)
  • C-peptide levels
  • Faecal elastase test (to assess enzyme function)
  • Autoantibody testing (to rule out Type 1 Diabetes)

Treatment

Insulin Therapy

Most patients with pancreatic diabetes require insulin as the primary treatment, since the pancreas cannot produce adequate insulin.

Pancreatic Enzyme Replacement Therapy (PERT)

Enzyme supplements to aid digestion and absorption of nutrients.

Dietary Modifications

A low-fat, nutritious diet tailored to the individual's digestive capacity and blood sugar goals.

Management of Underlying Condition

Treating the root cause (e.g., pancreatitis, cancer) is central to overall management.
